A neutral injection tracker template

A blank record structure you can print or copy, with no example protocol.

A useful personal tracker separates what was planned from what you recorded. Use one row for the date and time, your own label, the value exactly as supplied, an optional site label and a factual note. Leave medical decisions and changes to the licensed professional responsible for the treatment.

Keep planned and recorded actions separate

A planned date is not proof that an action happened. Mark a row only when you are recording what you chose to record, and keep a missed, skipped or changed entry distinct from the original plan.

If you correct an earlier row, retain a short correction note instead of silently replacing the history. A readable record shows what changed without claiming why the change was medically appropriate.

Use labels that make sense later

Choose a stable label you will recognise without adding unnecessary personal detail. Copy any value and unit exactly from the record you are using; do not convert units inside this template.

An optional site label can help distinguish your own entries, but this page gives no site-selection or injection-technique guidance. Ask a qualified professional when you need instruction rather than turning a blank field into a recommendation.

Move the structure into a private app if it fits

Pinned uses a structured personal record on iPhone and can place an optional Face ID lock in front of the app. It also supports encrypted backup exports that you keep yourself. It does not require a Pinned account for the health record.

A digital record can make entries easier to find, but it does not turn the template into medical advice or verify the content you entered.

This blank record does not recommend an amount, interval, product, technique or protocol. Nothing on this page is medical advice.

Common questions

What belongs in an injection tracker template?
Use the date and time, your own label, a value copied exactly as supplied, an optional site label and a short factual note.
Does this template include a protocol?
No. It contains no example amount, interval, technique or named product and does not recommend a protocol.
Should a planned row count as completed?
No. Keep the plan separate from what you recorded. A scheduled date does not prove that an action happened.
How should I correct an entry?
Keep the original context and add a short factual correction. Do not silently rewrite the record in a way that hides the change.
Can Pinned store this kind of record?
Pinned keeps a structured personal record on iPhone, with an optional Face ID lock and encrypted backup export. It does not verify medical appropriateness.
